With the introduction of CFSS, many individuals may not fully grasp how it functions or how it differs from the PCA program. Professional Service Group aims to ensure that every individual gains a thorough understanding of what Community First Services and Supports (CFSS) entails, the key distinctions between CFSS and PCA, who qualifies for CFSS, and the range of services available under this program.

Developing a Personalized Plan

Once the individuals are well-informed about CFSS and the various models available, we work alongside them to create a tailored plan that addresses their specific needs and outlines the reasons they require CFSS. We assist this process by providing templates and forms that make planning easier.

Fostering Community Engagement

At Professional Service Group, we place a strong emphasis on community involvement and helping wherever possible. We believe a thriving community leads to more effective day-to-day planning. Our consultants regularly share information on local activities and resources with the person, their guardian, and other household members.

Monitoring and Enhancing Service Quality

We assess the quality of our services through regular surveys, which help us identify areas for improvement. In addition, we engage in ongoing conversations with both the individual and their guardian. Based on the feedback from these interactions, we adjust plans and approaches as necessary. This ensures we continually enhance the quality of our services and improve overall individual outcomes.

Training and Assessing Our Staff

Our staff undergo comprehensive training in CFSS protocols and the use of various plan-writing tools. Communication and teamwork are also highly prioritized. By reviewing survey feedback and performance evaluations, we can identify areas where staff training needs to be strengthened. Our training steps include:

  • Completing online modules
  • Attending in-person sessions
  • Submitting required training documentation

What Community First Consultation Services Provides

Professional Service Group offers guidance to participants and their representatives when goods or services are denied. Common reasons for denial include:

  • Services or goods that are outside the scope of CFSS
  • Items not connected to an assessed need
  • Services or goods in the plan that don’t promote independence or reduce reliance on human assistance.

When necessary, we work with the participant’s Care Coordinator or Case Manager to address any plan reductions or denials. Upon request, during a change in condition, or in coordination with DHS or the lead agency, we help participants revise their plans by:

  • Allowing unlimited revisions
  • Ensuring the changes are in line with CFSS guidelines
  • Updating relevant goals as needed
  • Distributing the updated plan to the appropriate agencies and FMS
  • Sharing updates with the Care Coordinator or Case Manager, if required.
